ABOUT US
The Montgomery Area Community Wellness Coalition is a community-managed, nonprofit organization of health and human services providers who share information and coordinate services to increase quality, efficiency and effectiveness of services within the River Region. These public, private and faith-based providers came together in 1999 in Montgomery, Alabama to seek solutions to reduce economic and health burdens due to lack of health insurance. They incorporated in 2002 as the Montgomery Area Community Wellness Coalition for the following purposes:
- Identify and quantify the needs of the medically uninsured and underinsured population in the Montgomery, Alabama area
- Strengthen the health services infrastructure by coordinating the efforts of various agencies providing services to this medically uninsured and underinsured population
- Increase access to appropriate services for improving health and wellness
This group has recommitted their attention and effort in 2005 to expand their efforts to the entire River Region so that quality services are available and accessible for everyone to improve their health status and prevent or control diabetes, asthma and obesity through:
- Improving appropriate health and social services use through easier access with a single call to 2-1-1, community education, and wellness case management; by sharing information to improve provider healthcare and system decisions; and by providing quality improvement feedback to providers
- Increasing efficiency through maintaining an electronic, shared information database to help avoid duplication and fragmentation among providers and to make available single access eligibility determination and referral by Wellness Adovcates for other programs that target services to meet client needs; and through increased outreach to identify additional persons at risk and link them with wellness case management or other preventive services as needed
- Improving coordination of services through the sharing of information among providers to identify and implement service and system changes, or repair gaps and improve provider capacity; through wellness case management servcies that provide education and information for both clients and providers that will improve services use, and by maintaining accountability to the community and groups who share common goals and/or who want to learn from our model.
Our Mission
To conduct and support activities and services that improve health and wellness through coordination and information-sharing; health promotion and disease prevention; and by providing wellness case management services that help individuals and families at risk of or having diabetes, obesity or asthma to make healthy lifestyle changes, and/or to find and use health services and community resources.
